Forms, surveys and reports for all functions of CSMC.  These are the same items we have been filling out for years, but now they are all listed on one page.  If you have made Favorites or Bookmarks of these pages they are still good, this listing does not change the CSMC web site address (URL) of the individual pages.
 

Attendance at Meetings and Activities: (Comments about whether you are going and a report listing those attending and their activities.  The Round Robin is optional, replacing a traditional verbal event, outlining personal, family and corporate events you wish to share.)

Profiles:  (Required corporate and personal information from each member, associate and prospect.  These are blank forms on the CSMC web site.  The actual information you supply is distributed only to members and associates in a printed notebook and, for security reasons, is not available on the Internet.)
